You will need the following information to complete the documents for Expedited Material Transfers:

  1. Identification of material you are requesting

  2. Name of scientist at NIDDK who can provide the material (may be derived from a publication)

  3. Brief description of research use intended for the material (not required in all instances - see applicable document to determine if needed)

  4. Your contact information, including delivery address for material, e-mail address and phone number (Or, if you are a student or postdoctoral fellow, provide all contact information for your advisor or supervisor as this person will be required to sign on your behalf.)

  5. Name and contact information for the technology transfer professional at your institution, including name, mailing address, e-mail address and phone number

  6. Name and title for the official authorized to sign MTAs on behalf of your institution
